Submit this form to start the removal process. Once this form is submitted you will be able to search for your record, and a there will be a red 'Remove My Record' button next to your title. If you do not see the removal button you are either on the wrong page or your session has expired... in which case you should return to this form and start over. Once you click the removal button your record will be removed within 24 hours.
